Personal Support:

Provides participants that live with a family with support. Participants are provided with a staff member to spend time with both in the community or at home. This program is independent from their Work/Day Program schedule, and gives participants the opportunity to access the community, work on daily living skills, or even just participate in their favorite recreational activities.

Personal support services include:

  • Companionship in the home
  • Recreational activities in the community

In-Home Support:

Provides participants that live on their own, or with a roommate, with a staff member assigned to visit the participant’s home to aid with daily living tasks. Participants also can go out into the community and enjoy recreational activities. The overall goal is to support the individual’s life so that they may continue to live as independently as possible. In some cases, the Individual and Family Support Manager will assist with Title XIX redeterminations, matters of probate court, and securing entitlements.

  • Budgeting
  • Shopping
  • Bill paying
  • Attending doctor’s appointments
  • Recreation
